Thank you for expressing interest in having your child baptized at Our Lady of Perpetual Help!
The steps to baptism at Our Lady are simple!
- If you haven’t already, please register as a member of the parish.
- Fill out the Baptism & Beyond Parent Formation Form below.
- Meet with the office staff responsible for Baptism who will reach out to you after receiving your Formation Session registration.
- Choose sponsors/godparents based on the guidelines provided. Obtain letters of good standing from their parish if it is not Our Lady. This must be completed in order to schedule the baptism.
- Attend the Baptism Formation session.
- Register for the sacrament using the information received upon completion of steps 1-5.
